Description:

Provides multiple on-going assistance programs. Provides services and information including counseling, advocacy, information and referral, housing lists, limited transportation and support groups. Also builds wheelchair ramps, provides mobility equipment loan, maintains attendant care lists and provides skills training. The Community Living Services Program offers home modification, such as wheelchair ramps and adaptive equipment, and transitional services to assist persons with disabilities to leave institutions and return to living in their communities. This program also serves individuals at risk of being institutionalized.
