The Martinsburg VA Home-Based Primary Care Program delivers comprehensive, interdisciplinary primary care in the homes of veterans with complex medical, social and behavioral conditions for whom routine clinic-based care is not effective. HBPC provides primary care in your home. This means that the physician or nurse practitioner and the health care team are responsible for meeting your basic health care needs. You still will attend specialty appointments at the Martinsburg VA Medical Center. The HBPC team consists of: Physician/Nurse Practitioner, Registered Nurse, Physical Therapist, Dietitian, Social Worker, Clinical Psychologist, Pharmacist, Medication and Supplies: Many of the supplies and medications that you will need to help with your care will be provided to you by the VA Medical Center. You may have a copayment for medication and/or supplies depending on your veteran category (annual means test). Lab Work: Your HBPC nurse or practitioner will draw lab work at home and will transport it to the Medical Center for completion of testing. Medical Equipment: All medical equipment will be ordered by your HBPC team. The HBPC team also will provide instruction on how to safely use the equipment and how to care for it. Copayments: Copayments are based on your annual means test. If you have a copayment for outpatient primary care, you will pay a copayment for basic HBPC team visits. Regular lab draws are free of charge. A referral is necessary to receive HBPC services.
