Description:

Provides Case Management and Personal Attendant services through the Title XIX Aged & Disabled Program. The Aged & Disabled Program is defined as a long-term care alternative which provides services that enable an individual to remain in or return to home rather than receive nursing home care. The goals and objectives of this program are on providing services that are person-centered, that promote choice and independence, participant direction, respect, and dignity and community integration. Comforts of Home employs an agency Registered Nurse (RN) and Case Manager (Licensed Social Worker) to assist eligible persons in obtaining and maintaining in-home care services through the program.
