The Potomac Highlands Permanent Supportive Housing Program is designed to house individuals and families who are homeless and disabled while providing supportive services to aid in the transition from homelessness to permanent housing in Grant, Hampshire, Hardy, Mineral, and Pendleton Counties of West Virginia. Eligible individuals and families will benefit from intensive case management and supportive services with the goals of increasing self-sufficiency and independence and becoming a contributing member of society.
